Rules for the processing of personal data

Purpose and legal basis of data processing

We process your personal data only to the extent necessary to:

  • the provision of dental services,
  • keeping medical records,
  • arranging visits and contact with patients,
  • compliance with legal obligations (e.g. storage of documentation),
  • ensure safety and a high standard of care.

Legal basis for processing:

  • Article 6 (1) (b) GDPR — execution of a contract for the provision of medical services,
  • Article 6 (1) (c) of the GDPR — legal obligations incumbent on the Controller (e.g. storage of medical records),
  • Article 9 (2) (h) GDPR — processing of medical data for the purpose of diagnosis and treatment of the patient.

Scope of processed data

We collect only the data necessary for the provision of health services, such as:

  • name and surname,
  • phone number,
  • e-mail address,
  • health data (medical records),
  • PESEL (if required for documentation).

Recipients of data

Your data may only be transferred to entities that support us in the provision of services:

  • prosthetic laboratories,
  • entities operating IT systems,
  • an accounting firm,
  • hosting and IT service providers — with the highest security standards.

Data is not transferred outside the European Economic Area (EEA).

Data retention period

Personal data will be stored for the time necessary to achieve the purposes for which they were collected, in accordance with applicable regulations, in particular:

  • medical records — at least 20 years from the date of the last entry,
  • contact details — until the consent is withdrawn or the provision of services is terminated.

Your rights

You have the full right to manage your data. You are entitled to:

  • the right of access to data,
  • the right to rectify them,
  • the right to erasure (in justified cases),
  • the right to restrict processing,
  • the right to data portability,
  • the right to object,
  • the right to withdraw consent at any time,
  • the right to lodge a complaint with the President of the Office for Personal Data Protection (PUODO).

Is the data profiled?

We do not conduct automated decision-making or data profiling.

Consent to data processing

Providing data is voluntary, but necessary to make an appointment and provide dental services. You have the right to withdraw your consent at any time by contacting us.
